  • Comments (Metabolism): Prolongs QT interval and requires initiation in hospital; adverse events include torsades de pointes, HF, exacerbation of COPD/bronchospasm (K); renal dosing required if CrCl <60mL/min, contraindicated if CrCl <40ml/min
